On your first visit you will be scheduled for an hour with your therapist (one-on-one). We will discuss your problem, take a thorough look at you, and develop a plan for how to best treat you. If available, please bring your insurance card, prescription for therapy, and any other documents pertaining to your problem such as operative reports, imaging reports, or a doctor’s specific protocol to review.
The new patient form is necessary to treat you, and we have tried to make it as short as possible but still give us the needed information. The privacy policy and the fitness center release documents are only for your reference as you will initial you received them on the new patient form. The last step is choosing one of the four outcome studies that pertains to your problem based on body part; we use these to track your progress and help justify to insurance companies your continued need for therapy.
